This study analyzes the medical application of the Regulation on the Procedure for Voluntary and Involuntary Active Euthanasia in Ecuador, identifying the main challenges that limit its effective implementation. The objective was to examine the regulatory and clinical gaps of the regulation and to propose guidelines to strengthen its operability within the health system. A qualitative design was employed, based on documentary review of academic sources and critical analysis of international regulatory frameworks. The results revealed deficiencies in pharmacological guidelines and clinical protocols, lack of structured professional training programs, weaknesses in external supervision mechanisms, and absence of clear protocols regarding conscientious objection. These findings highlight the need to complement the regulation with technical instruments and monitoring mechanisms that ensure safety, transparency, and respect for patients’ rights. It is concluded that the effectiveness of the regulation depends on its articulation with concrete operational measures.
