Transforming the work environment in hospitals through advanced human talent management is essential to improving institutional efficiency and the quality of patient-centered care. The objective of this study was to design an advanced human talent management plan aimed at improving the work environment as perceived by physicians at the Municipal Hospital for Women and Children in Cuenca, Ecuador. The theoretical framework addressed strategic planning, participatory leadership, internal communication, professional development, well-being, and competency assessment. A non-experimental, mixed, cross-sectional design was applied, using structured surveys of 32 health professionals, analyzed with JASP and descriptive statistical tests and chi-square. The results showed a mostly positive work environment, good perception of leadership, communication, and professional development, as well as needs for feedback and workplace equity. It was concluded that a plan based on strategic axes strengthens cohesion, talent retention, well-being, and institutional performance.
